NHS Health Call is a joint partnership between County Durham and Darlington NHS Foundation Trust (CDDFT) and Inhealthcare Limited. Following the successful roll out of a number of digital health services, in 2013 we joined forces to create the NHS Health Call brand. The vision is to redesign care pathways across the UK, whether that is providing patients on anticoagulants greater freedom to manage their warfarin from the convenience of their own home or helping health professionals manage undernutrition at scale. Our award winning undernutrition programme improves the well-being of patients who are at risk of undernutrition, both in the community and in care homes. The service, which is live in dietetic clinics and care homes across the UK digitally monitors patient’s weight fluctuations, appetite and intake of oral nutritional supplements. The service acts as an early warning system, highlighting changes in health early on, whilst enabling more efficient prescribing of supplements. The service uses simple technology including automated phone calls or an online interface and integrates data into GP systems, meaning patient information can be viewed by dietitians, GPs and care home staff.
